Bangerter filters: partial occlusion explained

What a Bangerter filter is

It is a thin, self-adhering translucent foil applied to the spectacle lens in front of the better-seeing eye. Rather than removing that eye's image entirely, it scatters light and reduces contrast and detail by a calibrated amount. The amblyopic eye becomes the better eye for detailed tasks, which is the same mechanism as patching and atropine, delivered more gently.

Filters are supplied in a series of densities, labeled with numbers that correspond roughly to the acuity level the filter is intended to reduce vision to — from mild grades that take the eye down a few lines, through progressively denser grades, to versions that leave little more than light perception. The clinic chooses the grade, and the practical rule is that the filter must reduce the strong eye to slightly worse than the amblyopic eye for it to change anything at all.

Medical note: occlusion is a prescribed treatment. Which eye, how many hours and which method are decisions for your child's ophthalmologist or optometrist, and any change of method should be reported at the next visit.

The main advantages

  • Some binocular vision is preserved. The strong eye still sees, which matters for children who need to function normally all day.
  • No skin contact, which resolves the whole irritation problem.

The limits

  • Glasses are required, and they must be worn all day. No glasses, no filter.
  • The filter can be looked around, exactly like any lens-mounted occluder. A child who tilts their chin or turns their head gets an unfiltered view over the top of the frame.
  • It is partial by design. For dense amblyopia, even the strongest filters may not penalize enough to drive improvement, which is why patching remains first choice in severe cases.
  • Density labels are approximate. The actual reduction in a given patient depends on lighting and contrast, and clinical measurements often show less reduction than the label implies. This is why the clinic measures acuity through the filter rather than trusting the number.
  • Filters wear out. They scratch, cloud unevenly and peel at the edges, and they need replacing periodically.
  • They are usually fitted through an eye care professional, not bought casually online. The grade is part of the prescription.

What the evidence says

PEDIG compared Bangerter filters with patching in children with moderate amblyopia and found similar improvement in acuity over the study period. That is a meaningful result: for moderate cases, filters are a legitimate alternative rather than a compromise. Evidence in severe amblyopia is weaker, and standard practice there remains occlusion.

Filters are also used for reasons that have nothing to do with amblyopia — controlling intractable double vision in adults, and reducing nystagmus-related symptoms — where the aim is comfort rather than visual development.

How they are used in practice

  1. The clinic measures acuity in both eyes and selects a starting density that brings the strong eye just below the amblyopic eye.
  2. The foil is cut to the lens shape and applied to the back surface, where it is protected and less visible.
  3. Wear is full time, along with the glasses.
  4. Acuity is rechecked in the usual 6 to 12 week rhythm.
  5. As the amblyopic eye improves, the density is stepped down — and if the amblyopic eye stops gaining, density may be increased or the plan switched back to patching.

Filter, patch or drops?

A rough guide, though the decision belongs with your eye doctor:

  • Severe amblyopia, young child: patching, at the prescribed hours.
  • Moderate amblyopia, glasses worn full time, teasing is a factor: filters are an excellent fit.
  • Moderate amblyopia, no glasses: patching or atropine, since a filter has nothing to mount on.
  • Patch refused outright, skin reacting: filters or atropine, whichever suits the prescription.
  • Maintenance after successful treatment: a mild filter is a low-effort way to hold ground during tapering.
