Under-eye bags: what actually works

If the bag changes through the day, it is fluid and you can do something about it at home. If it is the same at 9am and 9pm, it is structural — and no cream, patch or roller will move it.

The question that decides everything

Does it change? Look in the mirror first thing in the morning and again in the evening, and take a photograph both times.

  • Worse in the morning, better by afternoon: fluid. Highly treatable at home.
  • The same all day, every day, gradually more prominent over years: herniated orbital fat. Not treatable at home.
  • Loose, crepey skin that folds rather than bulges: laxity. Partially treatable, slowly.
  • A soft mound on the upper cheek that worsens with salt and alcohol: a malar mound or festoon, which behaves differently from a true fat-pad bag and responds poorly to most treatments.

Fluid bags: what genuinely works

  1. Sleep with your head elevated. An extra pillow, or raising the head of the bed. Gravity moves fluid overnight; this is the highest-return change on the list.
  2. Cold, first thing. Chilled patches, a cool gel mask or a cold spoon for five to ten minutes constricts vessels and reduces swelling: caffeine patches.
  3. Cut the salt at dinner and the alcohol the night before. Both are reliably visible the next morning.
  4. Treat allergies properly. Allergic swelling is one of the most common and most missed causes, and antihistamines beat every cosmetic option for it.
  5. Stop rubbing. Friction worsens both swelling and pigmentation.
  6. Gentle lymphatic massage outward from the inner corner along the orbital bone. Modest but real, and free.
  7. Sleep on your back if you can. Face-down sleeping maximizes morning fluid.

Expect an improvement of the morning peak rather than the disappearance of the bag. If none of these changes anything at all, it was probably never fluid.

Fat-pad bags: the honest position

The orbital fat that cushions the eye is held back by a membrane that weakens with age. When it gives, fat bulges forward and creates a permanent bag. This is anatomy, not fluid, and it is why the bag is identical morning and night.

No topical product reaches it. Not caffeine, not retinol, not a jade roller, not a “bag-tightening” peptide serum — some of which produce a brief film-tightening effect that looks impressive in an advertisement and lasts an hour. What does work:

  • Lower blepharoplasty, usually with fat repositioning rather than simple removal, which fills the hollow below rather than leaving a scooped-out look. Often done from inside the lid with no external scar. Recovery and aftercare are in eye care after blepharoplasty.
  • Filler in the tear trough, which does not remove the bag but softens the step between the bag and the cheek so it reads as less prominent. Technically demanding and best done conservatively.
  • Good concealer technique, which for a bag means avoiding heavy product on the bulge itself — it emphasizes what it is meant to hide.

What is worth buying for the fluid type

Every morning

Caffeine hydrogel eye patches

  • Caffeine and humectants; stored in the fridge
  • 15–30 minutes per session
  • Cold plus vasoconstriction, which is exactly what fluid needs
  • Visible within a single session
  • Nothing at all for fat-pad bags

No consumables

Reusable cooling gel mask

  • Contoured gel mask kept in the fridge
  • Washable cover
  • Ten minutes of cold, endlessly repeatable, no cost per use
  • Useful for allergy flare-ups and after a bad night
  • Must not press on the eye — choose a contoured shape

Over months

Retinoid eye cream

  • Low-strength retinoid formulated for the eye area
  • Nightly, introduced gradually
  • The best-evidenced topical for skin quality and mild laxity
  • Also helps texture and fine lines
  • Irritating at first near the eye, and useless against fat pads

Things that do not work, despite the marketing

  • Hemorrhoid cream. An old trick that briefly tightens skin and risks genuine irritation next to the eye. Not worth it.
  • Cucumber and tea bags beyond the cold they provide. The cold is the active ingredient; a chilled spoon does the same job.
  • Face rollers beyond a temporary lymphatic effect that lasts an hour or two.
  • “Instant tightening” peptide serums, which form a film that shrinks as it dries. Genuinely dramatic for an hour, then gone, and prone to visible flaking under makeup.
  • Anything promising to remove fat pads without a procedure. There is no such product.
Worth getting checked: swelling that appears suddenly, affects one eye only, is painful, red or hot, comes with vision change or fever, or arrives alongside new fatigue, weight change or ankle swelling. Thyroid disease, kidney and heart problems, allergy and infection all present with periorbital swelling, and none of them is a skincare issue.

What to expect, realistically

The fluid component can improve substantially in a week of consistent changes. Skin quality improves slowly over months with a retinoid and daily sunscreen. Fat pads change only with a procedure. Knowing which of those three you are dealing with is worth more than any product in your bathroom cabinet — and if you are also unhappy with color rather than contour, that is a different problem again: types of dark circles.

Frequently asked questions

Can eye patches get rid of under-eye bags?

They reduce the fluid component, particularly first thing in the morning and when used chilled. They do nothing for bags caused by herniated orbital fat, which stay identical all day and only respond to a procedure.

Why are my bags worse in the morning?

Lying flat lets fluid pool in the loose tissue under the eye overnight. Sleeping with your head elevated, cutting salt and alcohol the night before and applying cold on waking all reduce the morning peak.

Does hemorrhoid cream work on eye bags?

It produces a brief tightening effect and carries a real risk of irritating the thin skin next to the eye. Chilled patches or a cold gel mask achieve more, safely.
